Imaging Tests Identify Role Of Allergies In Chronic Sinus Disease

Exposing patients with chronic sinus disease to allergens and then obtaining repeated images by X-ray or ultrasound reveals that nasal allergies may be involved in some cases of chronic sinus disease, according to a report in the December issue of Archives of Otolaryngology-Head & Neck Surgery, one of the JAMA/Archives journals.

Chronic disease of the maxillary sinus (the sinus cavity located in the mid-face beneath the cheeks, on either side of the nose) is common and affects a wide population of adults and children, according to background information in the article. "Although the involvement of hypersensitivity mechanisms, and especially of nasal allergy, in chronic disease of the maxillary sinuses has been recognized, the diagnostic procedures for this disorder and the relationship vary," the author writes. "There is a dearth of information regarding the direct causal involvement of hypersensitivity mechanisms of the nasal mucosa and potential consequences within the maxillary sinuses."

Zdenek Pelikan, M.D., Ph.D., of Allergy Research Foundation, Breda, the Netherlands, studied 71 patients with chronic maxillary sinus disease and 16 control individuals with allergic rhinitis but no history of sinus disease. The patients with sinus disease underwent a total of 135 nasal provocation tests, in which allergens were applied to the linings of their nasal cavities, and 71 control challenges in which only phosphate-buffered saline was applied. In the control patients, 16 positive nasal provocation tests were repeated. Before and repeatedly after these tests and challenges, images were taken of the maxillary sinuses using both radiography (X-rays) and ultrasonography. Changes to the skeleton, air fluid level, thickening of the mucus membrane in the sinus and other parameters were noted.

Of the 71 patients with sinusitis, 67 developed 104 positive nasal responses to the provocation tests. Of these, 89 were accompanied by significant changes to the maxillary sinus on radiographs and 83 were also associated with significant changes on ultrasonograms. No significant changes on radiographs or ultrasonograms were noted during the 71 saline control tests on patients with sinus disease, or during the 16 nasal provocation tests conducted on control patients without sinus disease.

"The possible involvement of allergy, and especially of nasal allergy, in some forms of sinus disease has already been reported in the literature," the author writes. "There are a number of anatomic and physiologic similarities between the nasal mucosa and mucosa of the maxillary sinuses." The maxillary sinuses open into the nasal canal through a valve known as the ostium. If mucus membranes in the nasal cavity are swollen, the ostium can become blocked, trapping fluids in the sinus.

"In conclusion, nasal allergy may be involved in chronic disease of the maxillary sinuses in some patients," the author concludes. "Nasal challenge with allergen combined with ultrasonography and, if necessary, also with one of the radiographic imaging methods may be a useful supplement for the diagnosis of this disorder in the clinical practice, especially in children. The confirmation of involvement of nasal allergy in patients with chronic disease of the maxillary sinuses would indicate an additional treatment of the nasal allergy."

Pete Townshend, el famoso guitarrista de la banda de rock británica, The Who, advierte a los jóvenes de los peligros de escuchar música en los MP3 y demás reproductores de audio digitales. Te recomienda que bajes el volumen de estos aparatos antes de que sea demasiado tarde para tu capacidad auditiva.
“La pérdida de audición es una cosa terrible porque no puede curarse. Si utilizas un iPod o un aparato de este tipo, o lo usa tu hijo, está bien, pero mi intuición me dice que tendrá terribles consecuencias en el futuro,” afirma Pete Townshead en su página web - www.petetownshend.co.uk.
Culpa de los auriculares
Pete Townshend, la leyenda del rock, a sus 60 años se quedó desolado cuando se enteró que padecía un problema auditivo irreversible. Sufre tinnitus, un ruido continuo en sus oídos, y una pérdida de audición progresiva, que le obliga a descansar 36 horas entre grabación y grabación, y le ha obligado a aplazar su gira mundial y otros proyectos.

Según el polémico guitarrista que solía destrozar guitarras en el escenario en los 60, su problema auditivo no se debe a la música a todo volumen de sus actuaciones con The Who, sino al uso de los auriculares del estudio de grabación.

Research To Investigate The Clinical And Cost-effectiveness Of Bone Anchored Hearing Aids

New research commissioned by the National Institute for Health Research Health Technology Assessment (NIHR HTA) programme will assess the clinical and cost-effectiveness of bone anchored hearing aids (BAHAs) for people who are bilaterally deaf (deaf in both ears). Hearing impairment is the most common sensory deficit and it is estimated that there are approximately 688,000 severely to profoundly deaf adults in the UK. According to evidence given to the House of Commons Health Committee, the NHS tariff puts the cost per person of conventional hearing aid provision from appointment and clinic time to use of equipment at about £270. The price of a working BAHA per patient is estimated to be at least £4500.

Most people with hearing loss can benefit from conventional air-conduction hearing aids, which work by amplifying sound passed along the ear canal to the eardrum making it vibrate. However, some people cannot wear these or do not benefit from them because the conduction process is obstructed in some way. Several clinical studies have demonstrated the efficacy of BAHAs in patients with conductive or mixed hearing loss. A BAHA delivers sound through the skull by vibrations, missing the outer and middle ears. Hearing loss can occur in both ears (bilateral), and in these cases, BAHAs are usually fitted on just one side (unilaterally). However, it has been suggested there may be benefits of bilateral BAHAs. The benefits and costs of bilateral compared with unilateral BAHAs, and of BAHAs compared with conventional aids or surgery is not known.

Researchers, led by Dr Jill Colquitt of the University of Southampton, will review existing evidence on the clinical and cost-effectiveness of: unilateral compared with bilateral BAHAs; BAHAs compared with conventional hearing aids; and BAHAs compared with surgery, for people who are bilaterally deaf.

"We propose to bring together the most up to date and highest quality published and unpublished evidence on the benefits, harms and costs of BAHAs for people who have hearing loss in both ears," says Dr Colquitt. "Provision is extremely patchy across the UK and BAHAs are not widely available. We hope that our research can aid decision making on the use of BAHAs and provide a more uniform approach which will benefit patients."

This INTERDISCIPLINARY course NEURO/ENT is organized by Prof. Bettag from the Neurosurgical Dept. at Barmherzige Brüder Hospital and his colleague Dr. Schäfer from the ENT Dept. at Mutterhaus der Borromäerinnen Hospital and will be held in Trier, May 13^th – 15^th, 2009.

A range of well know surgeons in this field like Dr. Gardner from the UPMC Kassam group and Dr. Cavallo from the Naples Cappabianca group has been invited to share their experience.

Este curso es el único en México que está reconocido por la Alexander Graham Bell Academy fro Listening and Spoken Language (antes AVI), único organismo que puede otorgar la Certificación en Terapia Auditiva-Verbal a nivel internacional, y fundado en 1981 por Daniel Ling, junto con otros pioneros de la TAV.
